In Short

• Hybrid (in-person and remote) care management duties.
• Assess member needs in various health areas.
• Oversee the development of client care plans.
• Connect clients to social services and supports.
• Advocate for clients with healthcare professionals.
• Utilize evidence-based practices in care.
• Conduct outreach and engagement activities.
• Evaluate client progress and update goals.
• Maintain patient health records in EMR.
• Complete monthly reporting for program compliance.

Requirements

• Active California Licensed Vocational Nurse (LVN) license.
• 2+ years of experience as a care manager.
• Knowledge of government and community resources.
• Excellent oral and written communication skills.
• Positive interpersonal skills required.
• Clean driving record and valid driver's license.
• General computer skills and knowledge of Google Workspace.
• Willing to work Monday-Friday 8:30am-5:00pm.
