• Oversee the development of client care plans.
• Offer services based on member accessibility.

Requirements

• Active California LVN license required.
• Experience as a care manager or navigator.
• Knowledge of government and community resources.
• Excellent communication skills.
• Positive interpersonal skills.
• Clean driving record and valid driver's license.
• General computer skills and knowledge of Google Workspace.
