In Short

• Supervise a team of approximately 10-15 Lead Care Managers.
• Oversee the creation of Individual Care Plans.
• Drive the team to meet key performance indicators (KPIs).
• Collaborate with Directors to implement changes and process improvements.
• Provide direct care management as needed.
• Travel to Los Angeles, CA on a monthly basis for up to 1 week.
• Additional travel up to 20-25% may be required.
• Work Monday through Friday with flexibility for evenings and weekends.
• Active LCSW license in California required.
• 2 years of people management experience preferred.

Requirements

• Willingness to obtain additional state licensure as needed.
• 2-3 years of care management experience.
• 1-2 years of post-acute experience.
• Proven leader with experience in change management.
• Strong organizational and independent working skills.
• Proficient in Microsoft and Google applications; Salesforce experience is a plus.
• Excellent communication and motivational skills.
• Working knowledge of lean Six Sigma methodology preferred.
