In Short

• The ECM Clinical Manager is responsible for Clinical Oversight and Management of ECM Lead Care Managers.
• Communicates with ECM Lead Care Managers and patients via telephone and telehealth.
• Guides crises encountered by ECM Lead Care Managers.
• Collaborates with the client’s broader care team for advocacy and service provision.
• Monitors team progress and coaches Lead Care Managers.
• Hybrid position requiring travel throughout LA County up to 2-3 times per month.
• Availability to work Monday-Friday, 8:30 am-5:00 pm (Pacific Time).
• Active RN license in California required.
• 2 years of people management experience preferred.
• Proficient in Microsoft and Google applications; Salesforce experience is a plus.

Requirements

• 2-3 years of care management experience.
• 1-2 years of post-acute experience.
• Strong organizational and independent working skills.
• Excellent communication and motivational skills.
• Working knowledge of lean Six Sigma methodology is preferred.
