In Short

• Assess member needs in various health areas.
• Oversee development of client care plans.
• Provide services where members are most accessible.
• Connect clients to necessary social services.
• Advocate for clients with healthcare professionals.
• Utilize evidence-based practices.
• Conduct outreach and engagement activities.
• Evaluate client progress and update goals.
• Arrange transportation as needed.
• Maintain up-to-date patient health records.

Requirements

• Willing to work Monday-Friday, with possible evenings/weekends.
• 2+ years as a care manager or community health worker.
• Experience working in a remote setting.
• Knowledge of government and community resources.
• Excellent communication skills.
• Positive interpersonal skills required.
• General computer skills and knowledge of Google Workspace, MS Office.
• Bilingual (English/Spanish) preferred.
