In Short

• Hybrid position requiring travel in San Joaquin County.
• Serve as the client’s primary point of contact.
• Manage client cases and coordinate healthcare benefits.
• Facilitate member access to care in a timely manner.
• Collaborate with caregivers and providers to promote wellness.
• Utilize evidence-based practices in care management.
• Conduct outreach and engagement activities.
• Maintain patient health records in EMR systems.
• Complete monthly reporting for program compliance.
• Attend training as assigned.

Requirements

• 2+ years experience as a care manager or similar role.
• Knowledge of social determinants of health.
• Clean driving record and valid driver's license.
• Excellent oral and written communication skills.
• Positive interpersonal skills.
• General computer skills and knowledge of Google Workspace.
• Bilingual (English/Spanish) is a plus.
