In Short

• Seeking an exceptional Lead Care Manager (LCM) to manage client cases.
• Coordinate health care benefits and facilitate member access to care.
• Collaborate with clients, caregivers, and healthcare providers.
• Conduct assessments in various health and social areas.
• Utilize evidence-based practices for client care.
• Engage in outreach activities to connect clients with services.
• Maintain documentation and health records.
• Hybrid position requiring field and remote work.
• Must have a clean driving record and reliable transportation.
• Bilingual in English and Spanish is required.

Requirements

• 2+ years of experience as a care manager or similar role.
• Knowledge of social determinants of health and community resources.
• Excellent communication and interpersonal skills.
• General computer skills and knowledge of Google Workspace and MS Office.
• Willingness to work Monday-Friday from 8:30am-5:00pm.
