In Short

• The Social Worker will be part of an Interdisciplinary Team (IDT).
• Provides support and care to patients and families.
• Conducts comprehensive psychosocial assessments.
• Provides grief and bereavement support.
• Addresses mental health concerns.
• Participates in weekly IDT meetings.
• Assesses and responds to psychosocial concerns.
• Facilitates advance care planning.
• Completes documentation within 48 hours.
• Assists with community outreach and education.

Requirements

• 2+ years of healthcare social work experience.
• MSW in Social Work or BSW with equivalent experience.
• Experience in hospice or palliative care.
• Palliative care certification preferred.
• Strong understanding of cultural competency.
• Excellent communication skills.
• General computer skills and knowledge of MS Office.
• Proficiency in EMR systems.
• Willing to work Monday-Friday 8:30am-5:00pm PT.
• Bilingual (English/Spanish) preferred.
