Requirements

• 2+ years of experience in care management or community health.
• Knowledge of social determinants of health resources.
• Excellent communication skills.
• Positive interpersonal skills.
• General computer skills and familiarity with Google Workspace and MS Office.
• Bilingual (English/Spanish) is a plus.
