In Short

  • Seeking an exceptional Lead Care Manager (LCM) to join our team.
  • Client’s primary point of contact for care coordination.
  • Manage client cases and coordinate health care benefits.
  • Collaborate with caregivers and providers to promote wellness.
  • Hybrid role with in-field and remote duties.
  • Assess and oversee development of client care plans.
  • Advocate for clients with healthcare professionals.
  • Utilize evidence-based practices for client engagement.
  • Maintain up-to-date health records and documentation.
  • Attend training as assigned.

Requirements

  • 2+ years experience as a care manager or community health worker.
  • Willing to work Monday-Friday, both in the field and remotely.
  • Knowledge of government and community resources.
  • Clean driving record and valid driver's license.
  • Excellent communication skills.
  • Positive interpersonal skills required.
  • General computer skills with knowledge of Google Workspace and MS Office.
  • Bilingual (English/Spanish) is a plus.
