• Seeking an exceptional Lead Care Manager (LCM) for a hybrid role in the San Francisco Bay Area.
• Serve as the primary point of contact for clients and coordinate with healthcare providers.
• Manage client cases and facilitate access to care and services.
• Conduct assessments in various health areas and develop care plans.
• Advocate for clients with healthcare professionals.
• Utilize evidence-based practices in care management.
• Maintain documentation and client health records.
• Attend training and ensure program compliance.
• Work Monday-Friday, both in the field and remotely.
• Bilingual in English/Spanish preferred.

Requirements

• 2+ years experience in care management or community health.
• Knowledge of government and community resources.
• Clean driving record and valid driver's license.
• Excellent communication and interpersonal skills.
• General computer skills and knowledge of Google Workspace and MS Office.
