Clinical Manager - Remote

Overview

The primary function of this role is to support quality patient care at Obran Health’s Home Healthcare Agencies in conjunction with the Care Team and Director of Nursing. You will leverage your experience in home health nursing to support field clinicians and direct home health services, ensuring that patients receive care in accordance with the agency's policies, procedures, and best practices. • Handling incoming calls from patients and staff regarding clinical questions.
• Collaborating with physicians and community agencies for coordinated care.
• Analyzing clinical data and identifying improvement opportunities.

• Supporting and educating staff for quality patient care.
• Providing oversight and care coordination for designated patients.
• After hours/on call responsibilities to support the team.
• Reviewing and signing clinical orders and documentation.
