In Short

• Fully remote position for a Registered Nurse (RN) with IL license and GI experience.
• Review patient vitals and care plans using proprietary technology.
• Establish and manage patient care plans addressing various needs.
• Respond to alerts in patient vitals to improve outcomes.
• Provide education and guidance via video, telephonic, and asynchronous encounters.
• Act as a liaison with physician clients and their patients.
• Coordinate care with physicians.
• Assist patients in developing healthy habits.
• Must have excellent customer service skills and be comfortable with technology.
• Work from home in a fast-growing startup environment.

Requirements

• Valid IL nursing license.
• Compact state license required.
• 3+ years of GI experience.
• Additional languages are a plus.
• Prior case management experience is a plus.
• Experience with motivational interviewing is a plus.
