In Short

• Monitor and review patient vitals and care plans using our proprietary technology platform
• Develop, implement, and manage care plans to address clinical, social, and behavioral needs
• Respond to alerts related to patient vitals and provide appropriate clinical guidance
• Deliver patient education and support through video, phone, and asynchronous communication
• Coordinate care with physicians and serve as a liaison between providers and patients
• Help patients build healthier habits through personalized coaching and support
• Maintain accurate documentation within the platform

Requirements

• Active and valid registered nurse (RN) or licensed vocational nurse (LVN) license (multi-state licensure preferred)
• Strong customer service and communication skills
• Ability to work independently in a remote, technology-driven environment
