• Seeking an exceptional Clinical Lead Care Manager (CLCM) for a hybrid role in Riverside County, CA.
• Primary point of contact for clients, coordinating with healthcare providers.
• Manage client cases, coordinate healthcare benefits, and facilitate access to care.
• Collaborate with caregivers and care teams to promote wellness and recovery.
• Assess member needs in various health areas and develop care plans.
• Utilize evidence-based practices such as Motivational Interviewing.
• Conduct outreach and engagement activities.
• Maintain up-to-date patient health records and documentation.
• Attend training as assigned.
• Requires travel throughout Riverside County.

Requirements

• Master's degree in Social Work (MSW) or active California LMFT license required.
• 1+ year of experience as a care manager or community health worker.
• Willing to work Monday-Friday, both in the field and remotely.
• Excellent oral and written communication skills.
• Positive interpersonal skills and a clean driving record.
• General computer skills and knowledge of Google Workspace and MS Office.
• Bilingual (English/Spanish) is a plus.
