In Short

• Monitor and review patient vitals and care plans using our proprietary technology platform.
• Develop, implement, and manage care plans to address clinical and social needs.
• Respond to alerts related to patient vitals and provide appropriate clinical guidance.
• Deliver patient education and support through video, phone, and asynchronous communication.
• Coordinate care with physicians and serve as a liaison between providers and patients.
• Assist patients in building healthier habits through personalized coaching and support.
• Maintain accurate documentation within our platform.

Requirements

• Active and valid registered nurse (RN), licensed vocational nurse (LVN), or licensed practical nurse (LPN) license in the state of Michigan.
• Bachelor’s degree in nursing or a closely related healthcare discipline.
• Strong customer service and communication skills.
• Ability to work independently in a remote, technology-driven environment.
