In Short

  • Seeking a Clinical Lead Care Manager (CLCM) for a hybrid role in Imperial County, CA.
  • Primary point of contact for clients, managing care coordination.
  • Collaborate with healthcare providers to assess and meet client needs.
  • Conduct outreach and engagement activities.
  • Utilize evidence-based practices in care management.
  • Maintain patient health records in the EMR system.
  • Complete documentation and reporting for program compliance.
  • Advocate for clients with healthcare professionals.
  • Assist clients with accessing social services and supports.
  • Attend training and maintain up-to-date knowledge in the field.

Requirements

  • Active California LVN license required.
  • 2+ years of experience in care management or community health.
  • Excellent communication and interpersonal skills.
  • Knowledge of social determinants of health.
  • Reliable transportation and clean driving record.
  • General computer skills and familiarity with Google Workspace.
  • Ability to work Monday-Friday from 8:30am-5:00pm.
