In Short

• Conduct telehealth visits to assess patients for palliative care eligibility.
• Manage patient care through regular assessments and updates to care plans.
• Oversee medication management and coordinate care with the interdisciplinary team.
• Facilitate advanced care planning and referrals as needed.
• Communicate with patients via phone and video conferencing.
• Complete all required clinical documentation to support program enrollment.
• Assume additional duties as directed by clinical leadership.
• Conduct outreach to community partners to develop relationships.
• Float nurse duties include assisting in coverage of patient care.
• Work remotely with palliative care patients wherever they reside.

Requirements

• Current California Registered Nurse license, unrestricted.
• Minimum of one year of experience as a professional nurse within the previous three years.
• Excellent oral and written communication skills required.
• Positive interpersonal skills required.
• General computer skills and knowledge of MS Office required.
• Willing to work Monday-Friday 8:30am-5:00pm in Pacific time zone.
• Ability to obtain licensure in other states including Oregon, Washington, Idaho, Utah.
• Experience in hospice, palliative care, oncology or home health admissions preferred.
• CHPN certification or equivalent is a plus.
