Case Manager - Remote

Overview

As a Case Manager, you will evaluate member needs to help them achieve optimal wellness by coordinating services and resources, while also addressing barriers to effective care.

Requirements

• Bachelor's degree in health or human services.
• 2+ years of related experience.
• Knowledge of Microsoft Office applications.
• Exceptional verbal and written communication skills.
• Experience with Medicare and Medicaid recipients preferred.
• Ability to use electronic information applications.
• Passion for improving consumer experiences.
• Knowledge of community health agencies.
• Prior experience in health promotion and coaching.
• Valid driver's license required.
