• Seeking an exceptional Clinical Lead Care Manager (CLCM) for San Joaquin County, CA.
• Acts as the primary point of contact for clients and coordinates their care.
• Manages client cases and facilitates access to care in a timely manner.
• Collaborates with caregivers, providers, and the Care Team.
• Utilizes evidence-based practices in care management.
• Conducts outreach and engagement activities.
• Evaluates client progress and updates care goals.
• Maintains patient health records and documentation.
• Hybrid position requiring travel within San Joaquin County.
• Must have an active LVN license in California.

Requirements

• Active California Licensed Vocational Nurse (LVN) license required.
• 2+ years of experience as a care manager or similar role.
• Knowledge of social determinants of health and community resources.
• Excellent communication and interpersonal skills.
• Clean driving record and reliable transportation.
• General computer skills including Google Workspace and MS Office.
