Case Manager - Remote

Overview

Our team is seeking a Case Manager to work with us in Florida.

In Short

• Full-time position with hours from 9 am to 5:30 pm Eastern Time.
• Conduct outreach, provide education, and drive member enrollment.
• Create Service Plans with members based on their diagnoses and care gaps.
• Build measurable goals to help members manage challenges and reduce hospital admissions.
• Coordinate community resources to support members in achieving their goals.
• Act as an advocate between the care team and members.
• Schedule appointments and follow-up services for members.
• Report on hospital diversions and productivity outcomes weekly.
• Maintain required records and diagnostic reports.

Requirements

• Bachelor’s Degree in nursing, social work, or related discipline required.
• Experience in case management or health/behavioral health preferred.
• Certification as a Case Manager or Community Health Worker preferred.
• Knowledge of local resources and social determinants of health required.
