FL Case Manager - Remote

Join our growing team! We’re looking for a dynamic Case Manager to make an impact in Florida.

In Short

• Conduct meaningful outreach, provide education, and drive member enrollment.
• Create a Service Plan to guide engagement based on member needs.
• Build measurable goals for members to manage challenges and reduce hospital admissions.
• Coordinate community resources for each member.
• Act as an advocate and liaison between the care team and member.
• Schedule appointments and follow-up services for members.
• Report critical incidents and quality-of-care concerns.
• Prepare and maintain all required records and reports.

Requirements

• Bachelor’s Degree in nursing, social work, or health and human services.
• Experience in case management, health, or behavioral health preferred.
• Experience as a Certified Case Manager (CCM) or similar roles preferred.
• Knowledge of local resources and social determinants of health required.
