In this dynamic role, you will collaborate closely with dedicated colleagues to deliver exceptional care and empower our members to thrive in their daily lives.

In Short

• Provide comprehensive social service care management to frail elders and adults with disabilities.
• Assess members to develop a comprehensive Member Care Plan (MCP).
• Coordinate services that address members’ health and safety needs.
• Collaborate with a Field Care Nurse (RN) to update care plans.
• Conduct face-to-face assessments and maintain contact with members.
• Identify and arrange support services for members.
• Engage in clear communication with members.
• Maintain accurate documentation of activities.
• Foster collaboration with service providers and community partners.
• Travel necessary for member visits and role responsibilities.

Requirements

• Bachelor’s degree in human services or related field with relevant experience.
• Intermediate computer proficiency, including Microsoft Office.
• Experience with electronic case note documentation.
• Knowledge of community health and social service agencies.
