Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system through medical record review and consultation.

Requirements

• Passion for delivering care to patients with chronic illnesses.

• Current RN License; BSN preferred.
• 2+ years of RN experience with healthcare performance coaching.
• Current CPR or BLS certification.
• Intermediate computer skills; proficiency in MS Word, Excel & Outlook.
• Willingness to travel within an assigned territory.
• Spanish-speaking preferred in some markets.
