As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system through medical record review and consultation, identifying their needs and implementing interventions.

Requirements

• Current RN License required, BSN preferred.
• Minimum of 5 years RN experience.
• Current CPR or BLS certification.
• Willingness to travel within an assigned territory.
• Home office with internet connectivity required.
• 2+ years of Case Management experience preferred.
• 1+ years of utilization management experience preferred.
