Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, identifying their needs, and implementing care plans.

In Short

• Full-time RN Case Manager position.
• Hybrid-Remote work with local travel for in-person visits.
• Monday to Friday schedule, no nights or weekends.
• Coordinate care for patients with chronic illnesses.

• Minimum 5 years RN experience required.
• Intermediate computer skills required.

Requirements

• CPR and BLS certification required.

• Private home office with internet connectivity required.
• Preferred: 2+ years in Case Management.
• Preferred: 1+ years in utilization management.
