DaVita Integrated Kidney Care is seeking a Full-Time RN Case Manager to support complex patients in navigating the healthcare system, working in a hybrid remote capacity.

• Full-Time RN Case Manager position.
• Hybrid remote work with in-person visits required.
• Support patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
• Coordinate care and manage complex patient needs.
• Flexible Monday to Friday schedule.
• Must live locally for in-person visits.
• Identify social determinants of health affecting patients.
• Requires current RN license in KY and IN.
• Minimum 5 years of RN experience required.

Requirements

• Current RN License in KY and IN.
• Minimum of 5 years RN experience.
• Current CPR or BLS certification.
• Willingness to travel within assigned territory.
• Intermediate computer skills including MS Word, Excel & Outlook.
• Home office with adequate internet connectivity required.
• Preferred: 2+ years in Case Management.
• Preferred: 1+ years in utilization management.
