RN Case Manager - Hybrid/Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, identifying their needs, and implementing appropriate interventions.

In Short

• Support complex patients with CKD/ESKD.
• Work autonomously and collaboratively with healthcare teams.
• Conduct medical record reviews and consultations.
• Coordinate quality and cost-effective care.
• Flexible Monday-Friday schedule.
• Build empathetic relationships with patients and families.
• Identify and address social determinants of health.
• Assess patients for unreported medical conditions.
• Requires RN License and 5 years of experience.
• Home office with internet connectivity required.

Requirements

• Current RN License required, BSN preferred.
• Minimum of 5 years RN experience required.
• Current CPR or BLS certification.
• Driver's license and willingness to travel.
• Intermediate computer skills required.
• 2+ years of Case Management experience preferred.
• 1+ years of utilization management experience preferred.
