RN Case Manager - Hybrid/Remote

Overview

As a DaVita Integrated Kidney Care Registered Nurse (RN) Case Manager, you will support complex patients in navigating the healthcare system, implementing interventions based on their medical, social, emotional, and financial needs.

In Short

• Support complex patients with CKD/ESKD.
• Work autonomously and collaboratively with healthcare teams.
• Coordinate and facilitate quality care for patients.
• Conduct medical record reviews and consultations.
• Identify social determinants of health.
• Flexible Monday-Friday schedule.
• Must live local to Parsippany/Succasunna area.
• Full-time position with travel for in-person visits.
• Build empathetic relationships with patients and families.
• Manage care transitions and ongoing patient needs.

Requirements

• Current RN License in NJ required, BSN preferred.
• Minimum of 5 years RN experience required.
• Current CPR or BLS certification required.
• Intermediate computer skills required.
• Willingness to travel within assigned territory.
• 2+ years of Case Management experience preferred.
• Spanish-speaking preferred.
