Overview

Author Health is seeking a committed and empathetic Care Manager (RN) to coordinate care for adults with behavioral health needs, ensuring seamless transitions in care and improving patient health outcomes.

• Perform comprehensive assessments of patients' behavioral health and medical needs.

Requirements

• Current Registered Nurse (RN) licensure in Texas, Florida, or multi-state compact jurisdiction.
• At least 3 years of experience in behavioral health or medical case management.
• Strong understanding of behavioral health conditions and treatments.
• Excellent assessment and communication skills.
• Ability to work independently and collaboratively.
• Proficiency in electronic health records (EHR).
