RN Care Manager - Remote

Overview

As an RN Care Manager at Strive Health, you will play a crucial role in transforming kidney care by providing comprehensive support to patients with Chronic Kidney Disease and End-Stage Renal Disease. This position involves coordinating care delivery and optimizing patient outcomes through a hybrid model of telecommunication and in-person visits.

• Work closely with the care team to craft personalized care plans.

• Utilize clinical judgment and analytical skills in care delivery.
• Maintain accurate program software records.

Requirements

• Associate’s degree from an accredited school of nursing.
• Current Registered Nurse (RN) license in the state of practice.
• 3+ years of related education, experience, or certification.
• Current BLS or CPR Certification required.
• Efficient and reliable transportation for patient visits.
• Proficiency in Microsoft 365 Suite.
• Experience with electronic medical records (EMR).
• Strong patient and family relationship management skills.
• Demonstrated experience in proactive patient outreach.
• Proficiency in documenting and navigating EMR systems.
