Licensed Practical Nurse, Transitional Care Management - Remote

Overview

The Licensed Practical Nurse (LPN) works collaboratively with the care team to provide ongoing support and communication to chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD) patients. The LPN acts as a single point-of-contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up.

Requirements

• 5+ years of related education, experience, or certification.
• Active LPN Licensure in the state of practice.
• Reliable transportation and active driver's license.
• Internet connectivity with minimum speeds.
