Licensed Practical Nurse (LPN) - Chronic Care Manager - Remote

Overview

We are seeking a compassionate and patient-centered Licensed Practical Nurse (LPN) to join our care team as a Chronic Care Manager. This fully remote position plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education.

• Position Type: Full-time, 40 hours/week
• Schedule: Monday–Friday, no evenings or weekends
• Compensation: $22.00/hour (non-negotiable starting rate)
• Work Environment: Fully remote
• Active LPN license required
• Own Windows-based computer with webcam required
• Reliable home Wi-Fi connection needed
• Strong interpersonal and organizational skills required

Requirements

• Conduct comprehensive patient assessments to develop individualized care plans
• Collaborate with interdisciplinary care teams
• Monitor patient health status and facilitate follow-up care
• Educate patients and families about self-management and medications
• Maintain accurate documentation in compliance with standards
• Advocate for patient needs and coordinate services
