As a Transition of Care Associate, you will play a crucial role in helping members navigate their health care needs during significant changes in their health status, ensuring a seamless transition to their next care setting.

In Short

• Coordinate care for members post-discharge from hospitals.
• Conduct post-discharge assessments and ensure medication management.
• Engage with members, providers, and community organizations.
• Facilitate interdisciplinary care team meetings.
• Monitor members for alerts or changes in condition.
• Assist with securing community resources and supports.
• Utilize reporting to reduce emergency department utilization.
• Advocate for members who cannot advocate for themselves.
• Complete outreach calls and questionnaires within compliance timelines.
• Understand payer/plan benefits and articulate them effectively.

Requirements

• Active and unrestricted LPN license in state of residence.
• 3+ years of LPN nursing experience.
• 2+ years of discharge planning or hospice experience.
• 2+ years of experience with Microsoft Word, Excel, and Outlook.
• Clinical experience in areas like diabetes and CHF preferred.
• High School Diploma/GED required; Associate's or Bachelor’s in Nursing preferred.
• Ability to coordinate care and manage complex health needs.
• Excellent communication skills.
• Experience with care management programs.
• Knowledge of community resources.
