Overview

As a Transition of Care Associate at CVS Health, you will play a crucial role in coordinating care for members experiencing significant health changes, ensuring they receive the necessary support during transitions to new care settings.

In Short

  • Full-time telework position with a schedule from Monday to Friday, 8:00 AM to 5:00 PM EST.
  • Responsible for care coordination for members post-discharge from healthcare facilities.
  • Engage with members to ensure medication management and understanding.
  • Facilitate communication between members, providers, and community organizations.
  • Conduct assessments and monitor changes in member health status.
  • Assist with securing necessary services and supports for members.
  • Participate in interdisciplinary care team meetings.
  • Utilize reporting to reduce emergency department utilization.
  • Requires active LPN license and nursing experience.

Requirements

  • Active and unrestricted LPN license in the state of residence.
  • 2 years of LPN nursing experience required.
  • Discharge planning experience preferred.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Strong communication and interpersonal skills.
  • Ability to work independently in a telework environment.
  • Experience with care coordination is a plus.
  • Associate's or Bachelor's degree in Nursing preferred.
  • High School Diploma or equivalent required.
  • Knowledge of healthcare policies and patient advocacy.
