Overview

As a Transition of Care LPN at CVS Health, you will play a vital role in enhancing member care by coordinating health services for individuals experiencing significant changes in their health status.

In Short

• Conduct outreach calls and follow-ups as required.
• Work Monday to Friday, 8 AM - 5 PM EST.

Requirements

• 2+ years of LPN nursing experience.
• Active and unrestricted LPN licensure in PA or compact license.
• 1+ year of discharge planning experience.
• Proficiency in Microsoft Word, Excel, and Outlook.
• Strong organizational and multitasking skills.
• Effective verbal and written communication skills.
• Bilingual in English and Spanish preferred.
• High School Diploma or GED required; Associate's or Bachelor's preferred.
