RN - CA Licensure - Transition of Care Coach - Remote

Overview

The position is for a Registered Nurse (RN) who will work remotely to support Medicare/Medicaid patients transitioning from hospital to outpatient care.

• Active RN license in California required.
• 40-50% local travel required.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM PST.
• Collaboration with multidisciplinary teams to coordinate care.

Requirements

• Graduate from an Accredited School of Nursing.
• Bachelor's Degree in Nursing preferred.
• 1-3 years of relevant experience required.
• Active, unrestricted RN license in good standing.
• Valid driver's license and reliable transportation required.
• Preferred: 3-5 years of experience in case management.
• Preferred: Transitions of Care Sub-Specialty Certification or Certified Case Manager (CCM).
