Registered Nurse (RN) - Care Transitions - Remote

Overview

This position is for a Registered Nurse (RN) who will work with members and multidisciplinary teams to coordinate care and facilitate transitions for patients with high needs.

In Short

• Work with members throughout a 30-day program from hospital admission to discharge.
• Collaborate with hospital discharge planners and outpatient providers.
• Ensure safe transitions to appropriate care settings.
• Conduct telephonic visits for high-risk members post-discharge.
• Educate members on medication management and follow-up care.
• Use motivational interviewing techniques to support members.
• Assess barriers to care and provide necessary assistance.
• Facilitate interdisciplinary care team meetings.
• Provide consultation and education to non-RN case managers.
• Conduct medication reconciliation as needed.

Requirements

• Graduate from an Accredited School of Nursing.
• 1-3 years of experience in hospital discharge planning or home health.
• Active, unrestricted RN license in Texas.
• Valid driver's license with a good driving record.
• Bachelor's Degree in Nursing preferred.
• 3-5 years of experience preferred.
• Transitions of Care Sub-Specialty Certification preferred.
