Registered Nurse Care Coach - Remote

Overview

This is a remote role. CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program.

In Short

• Part-time role (20-25 hrs. per week)
• Follow and call patients monthly
• Provide education and coordinate care
• Close preventive care gaps
• Coach on self-management strategies
• Utilize specialized care management software
• Build rapport with patients
• Connect patients with community resources
• Conduct Transitional Care Management activities
• Encourage preventive care measures

Requirements

• Registered Nurse (RN) license
• Experience in chronic care management
• Strong communication skills
• Ability to work independently
• Tech-savvy with care management software
