Overview

CircleLink Health is looking for passionate, tech savvy nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program.

In Short

• This is a remote role.
• Part-time position (about 20-25 hours per week).
• Assigned a group of Medicare patients with chronic conditions.
• Provide education and coordinate care through monthly calls.
• Focus on strategies for self-management to keep patients out of the hospital.
• Excellent documentation and time management skills are required.
• Ownership of outcomes is essential for success.
• Utilize specialized care management software.
• Implement and improve the Plan of Care for patients.
• Conduct Transitional Care Management activities for high-risk patients.

Requirements

• Bilingual in English and another language.
• Licensed Practical Nurse (LPN) certification.
• Experience in chronic care management preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Detail-oriented with strong organizational skills.
• Proficient in using care management software.
• Commitment to high standards of patient care.
