LPN/LVN RPM Care Coach - Remote

Overview

As an RPM 365 Care Coach, you will engage patients, track and analyze their physiological data, provide education, and encourage adherence to their care plans in a remote setting.

• Serve as the primary contact for patients on the RPM365 platform.
• Assist with device setup and app connectivity.
• Engage with patients to ensure adherence to care plans.
• Schedule monthly check-ins and monitor patient data.
• Educate patients through shared resources.
• Notify healthcare providers of critical alerts.
• Attend virtual team meetings for updates.

Requirements

• Active nursing license that is multistate, compact, or Utah.
• Knowledge of chronic conditions and related monitoring devices.
• Proficiency with technology, computer platforms, and smartphones.
• Strong verbal/written communication skills.
• Comfortable with patient outreach and engagement.
• Highly organized, detail-oriented, and team-focused.

Benefits

• Salary: $45,000 - $49,000 (DOE) + performance bonuses.
