Care Coach - Remote

Overview

As a Care Coach, you will work remotely for 20-25 hours per week with a team of nurses to manage patients with chronic conditions enrolled in Medicare’s Chronic Care Management program.

• Educating patients on self-management skills and goal setting.
• Implementing and improving the Plan of Care.
• Utilizing Motivational Interviewing or other behavior change techniques.
• Conducting Transitional Care Management activities.
• Reducing care gaps by encouraging preventive care.
• Connecting patients with community resources.

Requirements

• Experience in chronic care management.
• Strong communication skills.
• Ability to work independently.
• Knowledge of motivational interviewing techniques.
