Care Coach - Remote

Overview

As a Care Coach, you will work remotely for 20-25 hours per week with a team of nurses to manage patients with chronic conditions enrolled in Medicare’s Chronic Care Management program.

In Short

• Educating patients on self-management skills and goal setting.
• Implement and improve the Plan of Care by updating medications and appointments.
• Utilize Motivational Interviewing or other behavior change techniques.
• Conduct Transitional Care Management activities for high-risk patients.
• Reduce care gaps by encouraging preventive care and chronic care management.
• Connect patients with community resources as needed.

Requirements

• Experience in chronic care management.
• Strong communication skills.
• Ability to work independently and remotely.
• Knowledge of motivational interviewing techniques.
• Understanding of healthcare systems and resources.
