Remote Care Coach

Overview

As a Care Coach, you will work remotely to manage patients with chronic conditions enrolled in Medicare’s Chronic Care Management program, contributing to the mission of shifting to preventative care.

In Short

• Work remotely for 20-25 hours per week.
• Educate patients on self-management skills and goal setting.
• Implement and improve the Plan of Care.
• Utilize Motivational Interviewing techniques.
