Registered Nurse Care Coach - Remote

Overview

CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare’s Chronic Care Management Program.

In Short

• This is a remote role.
• Part time role (about 20-25 hrs. per week).
• Provide education and coordinate care for patients.
• Follow and call patients monthly.
• Utilize specialized care management software.
• Build rapport with patients to improve health.
• Implement and update the Plan of Care.
• Connect patients with community resources.
• Conduct Transitional Care Management activities.
• Encourage preventive care measures.

Requirements

• Registered Nurse license.
• Excellent documentation skills.
• Strong time management skills.
• Ownership of outcomes and accountability.
• Experience with chronic conditions.
