Transition of Care Associate LPN - Remote

Overview

The Transition of Care Associate LPN is responsible for care coordination of members experiencing significant health changes, ensuring seamless transitions and facilitating post-discharge goal attainment.

In Short

• Full-time telework position.
• Schedule: Monday-Friday 8a-5p CST.
• Responsible for care coordination for members post-discharge.
• Complete post-discharge questionnaires and medication reconciliation.
• Monitor members for alerts or changes in condition.
• Facilitate interdisciplinary care team meetings.
• Engage with community resources and supports.
• Utilize reporting to reduce emergency department utilization.
• Advocate for members who cannot advocate for themselves.
• Requires active and unrestricted LPN licensure.

Requirements

• 2+ years LPN nursing experience.
• Active and unrestricted compact LPN licensure in state of residence.
• 3+ years of LPN nursing experience preferred.
• Effective verbal and written communication skills.
• Bilingual a plus.
• Ability to multitask and adapt to a fast-paced environment.
• High School or Equivalent - MUST be an LPN.
