The Transitions of Care Nurse Care Manager is responsible for managing patient care for those with kidney disease, ensuring they receive comprehensive and person-centered care post-hospitalization.

Requirements

• Associate in nursing degree required.
• Current RN License is required.

• Transitions of Care experience preferred.
• Chronic and complex care management experience preferred.
• 1 year of utilization management experience preferred.
• 3 years of clinical practice preferred.
• Intermediate skills with MS Office Suite required.
• Ability to work effectively in a remote environment.
