Care Coordinator - Remote

Overview

The Care Coordinator will support patients in the Pacific Time Zone by providing exceptional patient service and care coordination across disciplines, ensuring the best clinical care for patients living with kidney disease.

• Remote position supporting patients in the Pacific Time Zone.
• Responsible for patient engagement and enrollment.
• Develop and implement care plans under the direction of Nurse Care Manager.
• Conduct assessments to identify social determinants of health.
• Guide members in understanding and utilizing health and social services.
• Advocate for members to address barriers to care.
• Maintain communication with patients and healthcare teams.
• Ensure accurate documentation of member interactions.
• Provide educational resources to empower patients.
• Other duties as assigned.

Requirements

• 3 years of experience in healthcare coordination or community health.
• Experience with a team-based approach to care.
• Knowledge of community resources.
• Strong communication and teaching skills.
• Intermediate skills with MS Office Suite.
• Ability to work effectively in a remote environment.
