Care Coordinator - Remote

Overview

The Care Coordinator at Foresight Mental Health plays a vital role in bridging client care between our internal teams and external partners, ensuring timely and appropriate placement, enhancing care continuity, and promoting successful engagement in higher levels of care.

In Short

• Work a full-time, remote hourly position with a schedule Monday-Friday 9AM-6PM PST
• Serve as the primary point of contact for partnerships that require concierge care or closed-loop referral follow-up
• Facilitate internal and external referrals for Intensive Treatment Services (IOP and PHP)
• Track, manage, and document the lifecycle of all referrals
• Maintain strong knowledge of internal service offerings and eligibility requirements
• Communicate directly with clients for placement assistance
• Coordinate with therapists and psychiatry providers
• Develop and maintain relationships with external referral sources
• Use judgment and organizational protocols to guide referrals
• Maintain accurate documentation within EHR and tracking systems

Requirements

• High school diploma or equivalent required
• Associate’s degree in a healthcare-related field preferred
• 2–3 years of experience in care coordination or referral management
• Familiarity with higher levels of care and behavioral health systems
• Experience with electronic health records and case tracking systems
• Remote work experience is a plus
