Care Coordinator - Remote

Overview

As a Patient Care Coordinator at Wellbox, you will help patients manage their health journeys through telephonic outreach and individualized care plans.

In Short

• Manage patient care through scheduled phone conversations
• Document visits using technology platforms and electronic health records (EHRs)
• Develop care plans focused on physical, mental, and preventative health
• Coach patients through their treatment plans—including wellness, nutrition, and goal setting
• Help patients prepare for medical appointments and connect with resources
• Full-time position, 40 hours/week
• Strong tech skills are essential
• Orientation and training provided for the first 2 months
• Monthly bonus potential and referral bonuses available

Requirements

• Active Compact LPN license
• At least 2 years of clinical experience (care coordination preferred)
• Comfortable using EMRs, Microsoft Office, and other digital tools
• Strong communication and problem-solving skills
